Step 1: Choose the product that works best for you.


There’s a wide selection of products to choose from. And we make it a breeze to find the product that fits you.

Step 2: Complete and sign the easy application.


You can get our simplified, one-page application in two ways:


Step 3: Have a professional certifier complete and sign the certification.


Printed on the back of the application is a brief certification form. Just take it with you when you meet with a professional certifier, who can be any one of the following:

  • Audiologist
  • Physician
  • Physician’s Assistant
  • Advanced Practice Registered Nurse
  • Speech-Language Pathologist
  • Hearing Aid Specialist

The certifier will sign the form to confirm that your permanent disability makes it difficult to use the phone.

 

Step 4: Provide copies of these documents.


Please DO NOT send original documents. We only need photocopies of these very important documents to verify your identity and South Carolina residency:

  • Copy of a valid SC driver’s license, SC identification card, or SC voter registration.
  • Copy of a page from your current phone bill showing your phone number and address.

 

Step 5: Send everything in the envelope provided.


When we mail you the application, we include a pre-addressed envelope. Simply put these items into the envelope and mail it back to us:

  • Application Form completed with professional certifier signature
  • Copy of a valid SC driver’s license, SC identification card, or SC voter registration.
  • Copy of a page from your current phone bill showing your phone number and address.
